Hello, this is Dr. Foster. Urology Times reports that 5% of men visiting a urologist have chronic pain in their scrotum or testicles. In a country the size of India, that is nearly 4 million men!

Chronic pain can be caused by scar tissue or it may be referred from elsewhere in the sacrum, perineum, or abdomen.

Men, if you are experiencing pain in your scrotum or testicles it is critical for your health that you stop the pain. In almost every case where men have scrotum pain, they also have low testosterone. Low testosterone causes a host of male health problems such as ED, low libido, penile and testicular shrinkage, and less sexual sensation in your genitals. That's just the tip of the iceberg. Chronic low testosterone can cause heart problems, depression, lack of motivation, obesity, and many more health problems.

Here is why scrotum pain is so dangerous for men: It reduces your testosterone. If you are experiencing chronic scrotum pain, you understand how much it hurts. It can hurt at the level of childbirth. The reason it hurts so much is because of a little-understood nerve chemical called substance P. Substance P is found throughout your body, but no part of your body has as much as your scrotum. It is filled with substance P. Substance P is a pain magnifier. It makes a little pain feel extremely painful. That's why it hurts so much when you your testicles get injured. Substance P is trying to tell you to protect your testicles. Because scrotum pain hurts so much, it stresses your body. When your body is stressed, it sends cortisol to the area of your pain, which is the testicles. Cortisol does something very dangerous to men's testosterone. Let's look at all the ways it damages men:

  • Stress and pain cause your adrenal glands to focus on pumping out cortisol and sending it to where the pain signal is coming from (in this case, the scrotum). Normally your adrenal glands produce 5%-10% of your testosterone, but when they get busy pumping out tons of cortisol in response to pain, they do not have as much capacity to produce testosterone, so you lose that production.
  • Cortisol tells the brain to stop making Lutenizing Hormone (LH). LH is the hormone that instructs the Leydig cells (testosterone factories) in your testicles to produce testosterone. If cortisol turns off your LH, your body will produce very little testosterone.
  • When the testicles get flooded with cortisol, the cortisol competes with LH for receptors on the Leydig cells. The Leydig cells favor cortisol, so cortisol binds to the receptors and tells the testicles to stop making testosterone. It literally turns off testosterone production.
  • High cortisol causes your body to make more SHBG, which binds to the free-testosterone in your blood, leaving less testosterone for your body to use.

So, having pain in your testicles sets into motion a chain of events that can devastate testosterone production in your body.
